The whole thing is a cluster fuck. They had something like 1000 times as much ammonia nitrate as they were supposed to have on hand. Of course, inevitably OSHA (or whoever fines the shit out of them) will be pressured to make the fine small enough that it doesn't put them out of business.

Really OSHA has been all about perceived power for ages now. They just try and look kinda mean and hope everybody grumbling about big bad OSHA means nobody realizes they don't have that many bodies to do the work.

I don't know honestly. I just have anecdotes my brother told me from working on oil drilling and fracking operations out in Midland. Blatant OSHA violations everywhere but no one gave a shit because they knew OSHA was stretched too thin to do routine inspections and even if OSHA decided to show up they had to give some sort of ridiculous advance notice.
Wake up to find my sister's cat is salivating and drooling heavily. Put her outside. Consult Dr. Internet™ to find that it can happen temporarily when they eat something toxic, including licking toads, which we have lots of near the house.

Huh, I'm not sure about the notice thing. I know we've had inspections without any notice but they might have been from other places (UL or ATF) as I don't much get involved in that.
Even when OSHA shows up and finds some stuff the resolution is usually you fix it, the somewhat significant fine is reduced to 0 or some small nominal number if you were blatant about it. They make a note that shows up on a webpage.
Just how bad the work environment can be varies wildly. I've seen everything from places that look like some kind of modern utopia where everything stops if you come within five feet of a machine to places where they run unguarded high speed equipment and just kinda shrug if you ask about safety. Though the last was down south.
